sometimes they are there for days at a time and others they dissappear. I can feel them sometimes when my underwear irritates them. sometimes I have a strong butt smell other times I dont even notice them or again they dissappear for a couple days

Your description sounds like a mild form of hemorrhoids. Occlusive clothing doesn’t help much.
In terms of hygiene, every time people use the bathroom they usually use bathroom paper, I would strongly recommend wet wipes, this is if the person is not at his house. At their home I recommend to clean properly.
