Anyone had THD (transanal hemorrhoidal dearterialization)?

I have grade 3 internal hemorrhoids with some prolapse. I’ve had 3 ( CHR O’Reagan) bandings and not feeling much improvement. I don’t want a hemorroidectomy. I’ve read about the THD surgery and wonder if anyone has had success with grade 3 type?

I just had this procedure on 3/1/21 and currently on the mend. The pain from the THD with mucopexy comes from the involuntary anal clinching, which never got above a 5 out of 10 for me. Ice packs and pain meds/ muscle relaxers did the trick and it lasted roughly 10 days or so. My bigger issue was the constant urge to have a BM throughout the day, but that subsided a few days ago. A little bleeding still happens 1 out of every 4 BM’s for now, but my surgeon says that can occur up to and around 4 weeks. Overall the pain is very minimal and seems to be well worth it.

Hey Mary, I am 3.5 weeks post off from the THD surgery. I saw a colorectal surgeon and I made sure it was a colorectal surgeon who had done LOTS of these procedures before, rather than seeing a general surgeon.

I don’t know how many internals were inflamed but I had at least one grade 3 that prolapsed out every single bowel movement, despite me not having to strain 99% of the time (I drink 2 liters of water per day + metamucil fiber supplements but the hem damage was already done by the time I got on this routine).

One time I had a stomach ache and they couldn’t be pushed in at all. I went to sleep and thank God when I woke up they had gone back in, but that was scary. So mine were grade 3 and going onto grade 4 I guess. The one that prolapsed out every bowel movement was huge.

Anyways, 3.5 weeks post op and I haven’t had any signs of the hems. They haven’t prolapsed, I don’t feel them up there and also I had a stomach ache a few days ago with diarrhea and the hems STILL didn’t come out or prolapse. So, knock on wood, I think the surgery worked.

I never had the intense pain that everyone talked about. The pain was really intense the day of the OP after I got home. How I would describe it would be like the rubber bandings (I had 3 of them done before the surgery) but multiplied by 10. I had a slight burning feeling the first time I pooped but I pooped in a sitz bath for the week 1.5 weeks after surgery. That slight burning feeling was very manageable, I had felt worse pain with my grade 3 hems whenever I had a stomach ache.

There was a little bit of throbbing pain on my backside, but again I had felt worse throbbings when I had stomach aches with my grade 3 hems. I never got the “feeling of pooping burning hot glass” feeling that you read about online or on YouTube.

My biggest problem was having to pee. I had a lot of trouble with peeing the first week. The first time I peed post surgery was about 8 hours post surgery. I had to pee standing up in the tub with my feet in warm water, while running the shower faucet on a slight trickle to make it sound like a peeing noise. And I just peed in the tub. That was the only way.

I’ve read online where a lot of surgeons will make you pee in the hospital before you leave. I would not have been able to do that, meaning if they wouldn’t have let me go home then I would’ve had to have a catheter put in. And that probably would have made my experience 100X worse.

I’m not saying it was a walk in the park for me, but it was nowhere near the horror stories that I read about online. But everyone is different.

That’s my experience with it anyways. Hopefully the hems stay gone for good now!
