Recently, I noticed these spots, any idea about what could those be? Most of the spots showed up when I stopped my period of using izotretinoin. Could it be some sorts of side effects from declining/stopping the daily dose of izotretinoin or genital warts?? Also recently I have not had a good hygiene, maybe it could be acnes because of bad penis hygiene ? Please help.

not sure, doesn’t look like gw though. you should practice good hygiene, especially genital because bacteria can grow easily and complicate things.
