Balanitis or something worse?

I've been occasionally getting spots like this popping up for a week or 2 at a time then will be gone for a month or even 5 months before I get some new ones. doesn't hurt or itch and not really any pain when urinating. I had it swabbed last year when it happened for hsv and it came back negative. I'm just wondering if anyone could help me out. it'll form within hours, not sure if a correlation but it seems to happen if I've been sweaty for awhile without being able to take a shower.

What you are describing sounds very like you have a re-occurring yeast / fungal issue and this links with you not have a shower.
Unfortunately if this is what it is the spores that cause these kinds of issues are naturally present on the body, and obviously they tend to thrive in warm damp conditions such as the groin / genital region.
However, they can also get out of control almost anywhere on the body such as at the bottom of the back and under the armpits where again it is frequently sweaty and warm.
Most medical professionals would get you to use an antifungal on your penis for 14 days and see what the impact of that is. The real problem is if the fungus is out of control elsewhere on your body, even if it is only on your groin - the problem can pop up again days weeks or months down the line.
You may need to get a medicated wash and treat all the skin on your body to eradicate it.
In addition, if you are regularly getting sweaty doing activity of any kind, such as sports training, gym workouts, cycling for distances the fungus could be present on your sportswear. If this is the case ANY clothing for that matter that is in contact with your skin when it is sweaty would need to be washed at high temperature to kill-off the fungal spores.
