BPH Dutasteride Recovery Inconsistent

Hi Everyone,

Im 36 years old and have been diagnosed with BPH. my symptoms started about a week after a vasectomy. The urgency and incontinence were unbearable for about 2 months. long story short, I finally found a Dr who discovered I have an enlarged prostate. (Because of my age other doctors didnt think to look for it.

Anyway, Ive been on Dutasteride for 2 months and my symptoms have been doing better. i actually went 2-3 weeks where i felt “normal”! having said that, things have taken a couple steps back for the past week. have any of you experienced this- an inconsistent recovery? Thank you!

Please seek a second opinion from a urologist. I am struggling to believe that you have BPH at 36. You most likely have chronic pelvic pain syndrome. I can’t believe you been put onto such a strong medication.

in my experience with enlarged prostate you have some days better then other. probably has to do with eating habits. stress. Dutasteride does take up to 6 months to notice a real improvement.
I am 57 and have been taking it for about 5 years and it has helped but not a total cure.

Thanks for your thoughts. This actually is my 2nd opinion. i was on a plethora of antibiotics over the summer from my first urologist. i havent had any pain at all, just urgency and incontinence. New urologist dis a sonogram and found the enlarged prostate.

Thanks. yes, Due to the holidays, my diet has been crap and it got worse going back to work following vacation, which might be stress. Im curious, have you looked into surgical options?

No not yet and I hope not to anytime soon. what symptoms to you have other then to many bathroom breaks? prostatitas can cause inflammation. only asking because of your age.

Have you had a cystoscopy to look at your prostate directly? did your previous urologist not check your prostate at all? im 33 but I wonder if my urologist may have missed if I have a large prostate. he said it was small when he felt it and both my scopes by two different urologists said it was small too.

cialis helps me with symptoms also I started taking CBD oil which also helps. last year I dropped 10 lbs and that also helped in many ways. nutrition will be important going forward.

Its mostly urge to pee and light incontinence. its worse when sitting or laying down. Yes Ive had a PSA tests to check for inflammation. A Sonogram confirmed the enlarged prostate. it all started after a vasectomy which is strange.

Yeah Ive had a bit of everything. Cysto, urodynamics (which is awful), cultures and blood. My first doctor only did a cysto and had me on Bactrim for 6 weeks (it did nothing). Ive been told that while rare, Im just at the age where BPH can happen. The strange thing is that it happened after a vasectomy. literally over the course of a week i went from normal to not being able to sit down. Latest Dr said that I may have had an enlarged prostate and the surgery irritated something.

You might be suffering from chronic scrotal pain which is a nerve pain that originates in the spermatic cord nerve bundle that travels from the scrotum up through the epididymis and related plumbing and finally to the brain. Most urologists are not familiar with this condition so you need to request a consultation with a urologist who specializes in scrotal pain. Vasectomies have been implicated as a major cause of scrotal pain. The fact that you have an enlarged prostate is likely unrelated to this pain but BPH, if in fact you have it is also a serious, but separate problem and needs to be addressed though you have time for that one. I have both conditions as well and both can be managed well with the right doctor. Dutasteride is way too early a treatment for you and has long term bad consequences if taken for more than a year.

Thanks for writing Howard. Do you think it could be this scrotal cord condition if Im having no pain? Ive felt no pain, only the urinary symptoms.

Hmm. Yet your urodynamics test didnt show a blockage, and your cycstoscopy didnt show your prostate was enlarged? I would just be a bit skeptical if I am honest. If your prostate is enlarged it should cause your flow to slow down, unable to fully empty your bladder. I’d recommend that you look into post vasectomy pain syndrome as it sounds like the nerves have been annoyed or damaged. Your first uro would have spotted it with the scope and straight away wanted to prescribe you something or do surgery.

I have pretty good flow and my prostate is twice the size and maybe bigger than that even.
Do you have a family history of BPH?

Hey Jason- My cysto and uro did show obstruction. During the Uro i couldnt even pee while sitting down…

I apologize - I thought you were also having severe scrotal pain so if you are not having pain then it is unlikely due to what I wrote. Have you been checked for a UTI? Prostatitis is also a real possibility following a vasectomy even if a urine culture comes up negative. I round of antibiotics like Cipro might be warranted for you. Also have you been put on anti-inflammatories? These are simple things to try.

Yeah- I was on Bactrim for 6 weeks, Cirpo for 2 weeks and an anti-inflammatory for a while. good times had by all!

They both showed an obstruction? So your urologist saw you had an enlarged prostate with the scope but did nothing? Maybe you were right to get a second opinion then. Could you try flomax?

Do you know what your qmax is? Its strange as there is no history of BPH in my family. Both grandparents are in their 80’s and are fine in that regard.

Tried Flomax. Im now on Alfusozin and Myrabetriq as well. First doctor flew through the cysto. he was done in about a minute. He never checked for an enlarged prostate.

Second doctor is the director of the program at Weill-Cornell. knows his stuff. Its a weird sequence if events. While i trust him, I often wonder if the actual problem has been solved yet.
