Can someone tell me what are these white spots on the meatus ?

Hello, I have had those white spots on the meatus since a few years, but I stupidly never went to a doctor, there is one smaller on the bottom and a larger area on top, they get more or less white and visible depending on the day, but never expanded or caused me any discomfort. does anybody know what it could be and if i should be worry or treat it? thanks.

I would say they are normal white spots found regularly on the glans. If they have been there for years they are probably harmless.

James has covered what I would have initially have said. However, the second and third photos suggest you have really dry skin around your Meatus, so that it looks slightly wrinkled and some may even be loose / standing away from the skin underneath? (try rubbing a finger tip very gently over the skin in the area, then compare this with the skin on the lower part of the head of your penis?)
The Golden rule for healthy penile skin is to exclude ordinary forms of body wash, such as soaps and shower gels from getting onto the sensitive skin of the head / glans and the foreskin. Wash with water only and always make sure the skin is kept as dry as possible. This is particularly important after urinating: if you cannot wash use clean tissue paper / toilet tissue to throughly dry the area, then wash with water as soon as you can.
Unless the photos are giving the wrong impression I would say the skin on the head of your penis is already compromised: I can give you further information of the things you need to do and avoid - however, the important thing to know is whether you have problems with your skin elsewhere on your body … ?
