Can herpes 2 be spread to the shoulder or is it restricted to the bottom oonly?
Nothing is impossible, but that would be extremely unlikely and most unusual. Type 2 has a very strong preference for the genitals and doesn't really favour anywhere else.
When you say it favors the genitals? do you mean it prefers the sacral nerves versus the trigeminal near the face?

I had a couple of recent attacks on my right arm. A similar attack before that 6 months ago on my back tested positive for HSV2 (swab test). In my case I am hsv2 positive but thankfully don't get attacks on genitals but rather on my middle back, shoulders and more recently arm.
