Constant urge to urinate a year after HoLEP

It is almost a year since I had successful HoLEP surgery and for some time, things progressed well. However, I have had three bouts lasting a month or more each, where I have a constant need to urinate. It wakes me at night and I feel it all the time, it is very depressing.

I have had a cystoscopy and flow test just two months ago and it all seemed fine; no strictures, wide-open cavity.

The surgeon told me at this appointment that the involuntary sphincter had been cut so the only thing between my bladder and the outside world is the voluntary sphincter.

This latest round has been going on for a month and shows no sign of clearing up. I am on a list for urodynamics testing but am told it will take months.

I really don’t know what to do now.

Common causes for constant urge to urinate:

  1. Urinary retention. Bladder full but can’t empty.
  2. UTI
  3. Bladder/kidney stones

No, not retaining, no UTI either. Problem seems to stem from urethra on the external side of the voluntary sphincter and bladder empties just fine. I am doing pelvic floor relaxation exercises and have started taking fluoxetine to ward off the anxiety that comes from this problem.
