Damaged foreskin/glans and peeling

It all started around 2 years ago when I noticed a red spot on my glans and there was some foul smelling discharge. I visited a urologist and he gave me Balanil gel and just told me to wash my penis without any soap, just using lukewarm water. I used the gel for a while and the problem kind of went away, but whenever I masturbated the redness came back (discharge was gone for good though). This went on for like half a year until I finally decided to go to another urologist. Second one didn't see any kind of problems and just sent me home. Around that time the pandemic was in full swing and I was a lazy bastard so I didn't really go out much, not even to the doctors so the condition had worsened over time. My glans skin became really cracked if I kept my foreskin retracted for a few minutes (it was like leather snake skin, seemed like you could just peel it off) and the redness increased around the whole head. Also, small 2 white patches appeared on inner foreskin (under the penis head when you retract your foreskin) and whenever my penis was erect it seemed like there were tiny red bumps under the glans (or they were a part of my glans, I don't know). I did also try using coconut oil for over a month but I didn't really see any benefits to using it. So last spring I decided to go to a dermatologist. She prescribed me Travocort cream for 1 month daily usage and then twice a week for another month. This actually helped me really fast, it seemed to fix most of the problems in a few days (I kept using it as prescribed however, as she said if I stop using it instantly after it clears up it'll just come back again). My glans was still getting cracked and the tiny red bumps when erect were still there, but the soreness, redness and white spots went away. However a month or two after I stopped using the cream the problems started to come back slowly, so I went to another dermatologist (the previous one left on maternity leave, so I couldn't go back to her). He was surprised that I was given Travocort for such a long time and instead prescribed me Hydrocortisone for 5 days and Clotrimazole for 2 weeks. It didn't work as fast as Travocort did and after 2 weeks of usage it didn't seem like it helped me that much. The redness and the 2 white spots decreased, but my penis still didn't look quite right, and after a few days of stopping, it just became even worse than it has ever been, it looks absolutely horrible. The glans and inner foreskin are flaking and peeling off, they also look like they've been really damaged. I'm attaching images to show what it looks like right now. Is it a combination of severe dry skin and balanitis or something else entirely? I've made another appointment with the same dermatologist but it'll be in 2 weeks. Should I use something or just leave it be until my appointment? I'm genuinely lost and it seems like the problem will never go away, as it seems some people have been having problems with balanitis for years and still haven't found a way to properly manage it. I've read some tips from other people about diet (some say to cut out sugar entirely, others say just to eat meat and nothing else which seem a bit extreme for me), others say to stop having any sexual acts for a few months at least (even though those same people later reported that after masturbating/having sex for a few times just brought back the symptoms even after abstaining for months), and just general tips on how to wash your penis and what detergents to use for clothes. It's one of the few chronic conditions that I have and I'm honestly getting tired of trying to figure what's causing them, then trying to manage them and so on. Sorry for venting. TL;DR Had balanitis for 2 years. Tried a few ointments and tips from doctors/people but so far no good. Problem seems to have become even worse. Glans and inner foreskin are flaking and peeling, red, sore, white spots on inner foreskin, tiny red bumps under(?) glans.

Hi Man. sorry to read about your problems with your penis you have been through a lot. You definitely need to go back to your dermatologist from the photos it looks very irritated I don’t know what you have but maybe zoon balanitis and possibly lichen sclerosus. Has the foreskin adhered to the glans?

Yep, foreskin is really sticky, adhered to the glans. I haven’t used anything for a week now, just cleaning it once every day. Abstaining from masturbation as well since I don’t want to irritate it even further. It looks a bit better, but obviously far from how it should normally look like. Will see what the dermatologist says next week.
