As Lostknight suggests the best person to sort this would be a Dermatologist. Your regular Doctor / GP / Family Doctor can refer you to one, or you could go to a Genital Health Department at a General Hospital - most of these are referred to as GUM Clinics in the UK. Or, if you live elsewhere, check if you have a Specialist Sexual Health Walk-In Centre near you and ask to see the Duty Dermatologist or Urologist. Should be relatively straight forward to sort. Penile Hygiene Regime to Combat Specific Problems
The best approach is not to use ANY SOAP on the Glans / Head of your penis - wash with water only EVERY time you have urinated, after sexual arousal, and ejaculation. If you are not circumcised, protect the Glans of your penis every time you take a shower by drawing the foreskin as far forward as you can, then wash everywhere else, thoroughly rinse off all the gels / shampoo and suchlike, then expose your glans wash it with the water only using the inside of your hand, then each time you have washed it you must make sure to 100% pat dry immediately afterwards with a towel.
After you have put this in place and you still have the situation going on you need to look at what else your penis is coming into contact with on a fairly regular basis. The likely suspects are:-

I started washing my entire body in the shower with soap, and then only exposed the glans at last when all the soap was gone.
After washing my glans every time i urinated, every morning and evening, and every time i ejaculated, i can say that the symptoms have almost disappeared!
I don’t feel the itching anymore at all, and my glans almost looks how it used to do
So i deeply recommend just washing your glans as much as you can at every occasion possible!

Make this a regular part of your penile hygiene and in 3 months you should see a real difference! When you cannot wash, such as when you are out of the house, you can use some toilet tissue or clean paper hanky to wipe any traces of urine from the tip of your penis and wash later.
For sex or masturbation I keep a fresh toweling face cloth by the bed so that I can remove all traces of semen and pre-ejaculate from my entire penis rather than go to wash all the time. Buy a pack of faceclothes that only YOU would use for this purpose, then wash them in Liquid Soap Flakes on a regular basis …

For me it only took about 2 weeks, but i think it varies with every person, and how much you take care of it.
