yes boils are painful!!! i have a skin condition where i get alot (HS) they are tender and the swelling under can spread all around and hurt. take warm conpresses and put them on a few times a day to help bring to a head .

That looks like an infected follicle or an epidermal/inclusion cyst. Not herpes.
