Greatly reduced sensitivity/pleasure at head of penis, but can still achieve erections.

30 year old male, thin and in pretty good cardio shape. I’ve struggled with pelvic pain for years, which includes everything from pain in the lower back, rectum, scrotum, lower legs and feet. A pelvic MRI didn’t show anything unusual. I’ve never had a scan of my spine, but I’ve also never suffered a physical injury that would compromise the spine. The only thing noteworthy in my history is having chronic constipation and an anal fissure, which may have caused my pelvic muscles to spasm and compress a nerve.

Over time my sex drive has plummeted dramatically. My psychosomatic erections are almost virtually gone. Meaning I can’t think a sexual thought, or see something visually stimulating and get an erection. Those used to happen all the time, but now I don’t even flinch as much down there.

Through manual stimulation I can achieve erections. It takes longer, and there is about 80% less pleasure. The moment of ejaculation is pleasurable, but everything leading up to it is very muted in terms of sensitivity. Sometimes the pleasure is so non-existent that I just give up and find it not worth the effort. Also after ejaculation there will be a heavy ache deep in the perineum which can last several hours.

Does any of this give insight into whether my problem is coming from the spine or the pelvis? I’ve read that loss of psychosomatic erections can be a spinal problem, but maybe there are pelvic explanations for that too?
