Had problems with my shoulder for 2 years, need help!

I've been to a few physios, I've had many problems with my shoulders. I don't think my shoulders are rounded off really, however my right shoulder will still round off whenever I do a lot of stuff in the gym.

The main two things I struggle with now are shrugging my shoulders and lat pull downs. If you can imagine me hanging from a bar with a wide grip, and try to shrug my right shoulder blade won't sit in place. With normal shrugs my right shoulder blade seems to not be in place

This is so frustrating and getting me so down, if anyone could help it would be appreciated

Have you injured it in any way or form , 

i never used to train incline chest at all, only the upward movement doing shoulder press. I had rounded shoulders at the time and was lifting 4-8 reps. I ended up pulling my upper pec without knowing what was up I just carried on and avoided training chest

Then it just escalated since. It's all down to bad posture in every way. But I can't seem the fix this last bit

You need some massage treatment.
