What the doctor did say was right. Do not use soap in that area. Use warm water and pat dry do not rub

Matt, I think you are right to link poor hygiene and what you have now. I was a victim of this and did not realise the critical role of an impeccable penile hygiene regime when first noticing there was an issue. Although I realised in my early to mid-20’s I needed to wash after every time I urinated this was insufficient and I ended up with an incurable skin condition of the foreskin and head of the penis.
I am quite taken-aback by the lack of concern shown by the Doctor you have seen as they should have at least referred you to a Dermatologist.
The white band of skin across your penis below the head looks rather strange! I wonder if you ever suffered from Phimosis, that is, a tight foreskin that you may even have had to force apart when you were younger - perhaps teenage years?
Essentially what you need is to find a Dermatologist who deals with Genital Skin Disorders - you can do an internet search to see if you can find one in the area in which you live. Or, if you live within travelling distance of one of the larger Walk-In Sexual Health Centres you could contact them, or go along and ask for them to refer you to such a specialist.
In the mean time your penis needs to become a soap and any form of body cleanser (shower gels etc) free zone! Use water only and make sure you are completely dry. If you cannot wash, such as when you are out of the house use a clean paper tissue or toilet tissue to remove ALL traces of urine from the head and surrounding area of the penis. Ensure you are completely emptied out before putting your penis away as any leakage can cause a major issue, and simply further exacerbate what you already have.
