The spots are naturally present, what is likely going on is they being irritated by something and this is causing the red appearance. Sex and masturbation will cause further irritation, however, there is something that your penile skin is in regular contact with that is irritating it.
This could be one specific thing like a soap, shower gel, something you use as a lube or apply to your penis regularly, such as skin cream or moisturiser, or alternatively, it could be a combination of different things that are creating the irritation.
For starters, wash your penis with water only and fully dry afterwards …
