If the color in this photo is correct your perineum looks as if you have white patches on it. Have you seen a doctor lately? Were you officially diagnosed with AV or are you assuming you have it? I was diagnosed with it three years prior to being diagnosed with lichen sclerosis. One of the most common signs of LS is white patches on the perineum. See a specialists. If you do have LS you need to start using certain meds.
