How to I decide the treatment?

I had protected sex with a high risk CSW on 23rd Sep. Nothing unprotected except touching her genitals and deep kissing. Few days before the sex I was in a very bad hygiene and had masturbated frequently. I was sleeping and going after the usual things in the day my underwear in semen. The sex was very vigorous with lots of force used. After the act, I washed my penis with a detergent soap that is used to wash the cloths. I can't say for sure neither can I deny totally whether the discomfort in my penis started right after the act or next day or after couple of days. But on sixth day, the discomfort increased with an increase in urination and constant pain at the tip along with burning sensation and increase in urge to urinate. The doctor put me on  Cefixime course for 10 days starting after a week of the exposure. He also asked me to take Alkalizer until the burning stops and do a STD panel test. The symptoms seemed to have resolved over few days. In the meantime, my STD panel report came negative for HIV, Syphilis, Chlamydia for blood sample collected at 14 days after the act. Urine routine and culture came negative. An Ultra Sound Sonography report shows normal prostate and other organs but tiny renal calculi in left kidney, however, doctor said the report is normal. At 28 days, I tested for HSV Igg and tested positive for HSV 1 = 3.15 and negative for HSV 2 = 0.07. Enter the 4th week, I had fever, chills, soar throat. These symptoms seemed to have resolved after taking general medicines for fever and cold. However, my main symptoms of unexplained discomfort in the penis, burning, increase urination, mild pain at the penis tip keep recurring after few days. Doctor advised me to take plenty of water but it seems like taking a lot of water is aggravating my problem. And this happens mostly at day time. Did urine routine 3 times in two months and tested negative. 15 days ago I visited a urologist who put me on levofloxacin 500 twice a day for 5 days. At the end of the course I felt 80% improvement but because it was not 100 percent I decided to see the same urologist again. He was out of town so I consulted with another urologist who ruled out any STD and put me on 10 days of course of an alpha blocker. Again I felt much relief may be 90 percent. My alpha blocker course was over 1 day ago. Today I happened to drink lots of water for another ultra sound sonography and all the symptoms have come back with plenty of water in the bladder.

Right now:

1. Constant burning at the tip of penis for entire day.

2. Increase in urge to urinate with actual urine coming out.

3. Drank lots of water today and that clearly has aggravated the problem.

4. Feel mild pain and discomfort in penis during erection.

What is causing this issue? Is it the hard soap I used to wash my penis that has irritated the urethra? It has been two months now, could the soap effect be that long? A GP had prescribed me 20 days course of Levocetrizine 5 mg and candiForce capsules for the hard soap effect which I did not take as I was already on Alpha blocker. Another doctor of infectious diseases I met today prescribed me Norfloxacin for three days. Should I take this antibiotics? 

Well Not many responses. Sorry about that. Don't have any idea what to say.

I do know every minute of life we are making decisions. God made it that way with the "free will" thing he gave mankind. 

Hope I am wrong, but your message indicated your knew this situation before you had the sex with this woman? 

That being the case,  Bad Decision.   Wiithout protect suicide. With protection, very Bad Decision.

Sounds like a urine infection to me How does the uro know which antibiotic to prescribe? In the past I have been given antibiotics that had barely any effect, but after urinalysis the bug was identified and a different pill prescribed which cleared the problem in one week.

Too many toxic antibiotics prescribed.

It's easier said than done

BUT, find a competent urologist who doesn't have the Rx pad tied to his wrist.

Your primary will name one or two who based on your personality will be a match.

Don't give up, I went through 15 plus uros before settling on my current.

Thank you Pepasan and Sansrx fr the replies. My Urine Routine and Culture both rules out the infection. The sugar level in my urine too is in normal range. I did all the necessary tests yesterday at Infectious Disease specialist and he said all the reports are normal, however, he prescribed my Norfloxacin for 10 days which I have not started. I am completely normal at night. I also get perfect nocturnal erection. The symptoms starts as the day progresses with increase in intensity of constant burning at penis tip and urge to urinate.

May I know the bug that was identified in your urinalysis and the course of treatment you were given?

Okay. Great that the bug was identified in your case. Last night, after I return from the urologist I took the first dose of norfloxasin. The urologist wants me to go through Urethrogram imaging which I want to avaoid. I plan to finish the norfloxasin course and see if it can save me from Urethrogram.

This sounds silly, but I am serious. Is it possible that your problems could be caused by dirty underwear, or the soap used to clean them?

I asked this question to my urologist. He said, if that was the cause of my problem it can't be that long. Hard soap or bad hygiene can give problems for maximum two weeks and I am entering into my third month with these symptoms. Although the symptoms are not as severe as they were but they still persist. 

I would definitely get another option.
