I am very concerned, as he feel pleasure and his scrotum is full, but he can't seem to ejaculate. Can you please shed some light as to why this may be happening please? It has only developed in the last six months. His blood work as shown up clean. What can this be caused by?

We need a bit more information in order to help:
How old are you both?
Has he had this problem for a long time or has it come on recently?
Did the problem occur suddenly or has it crept up on you slowly?
Do you use condoms?
Can he ejaculate easily from masturbation?
Semen does not emit from the testicles. Most of the fluid containing the sperm (invisibly small but are formed in the testicles) is produced by glands higher up inside the man's body eg seminal vesicles, bulbourethral glands and prostate.
Therefore, the 'fullness' of his balls tells you nothing about his readiness to ejaculate. Ask him if he ejaculates when he masturbates. If so (and very likely) it's a matter of changing the type or timing of physical stimulus during intercourse or trying oral or manual stimulation for longer before attempting intercourse. Sometimes there are psychological factors. A sex therapist should be able to help you both find a way to make your sex life together satisfying for both of you.
