This looks most like a blood blister - it is very common for one of the many tiny little blood vessels to burst or leak particularly after masturbation, sex or having used any manual method of penile enlargement.
Only time will heal this - it does mean you will need to stop doing what may have caused this in the first place so that the blood vessel will heal and become strong again …
