Damian, the clue for what you have going on with your penis is that you state " it’s been like this for as long as I can remember " - and this is because this is how a penis should look.
That is, there is nothing unusual or to be concerned about with your penis.
Some tips on keeping penile skin as healthy as possible: when you take a shower draw your foreskin as far forward as it will go before you get in the shower - wash everywhere else on your body first then rinse of all cleansers you have used such as soap, shower gel and hair shampoo.
Then expose the head of your penis and wash it with the running shower water only, rub with the inside of your hand and fingertips to clean. NEVER use hot water for this or whenever you wash your penis.
Always make sure you fully dry your entire groin region, penis, head of penis and inner-foreskin.
