Hi, you need to see your doctor to have it checked and diagnosed properly. it could be a fungal yeast infection. But you need to get the phimosis sorted as well so you can clean easily that’s important.
Essentially, if you can expose that much of the head of your penis you do not have a severe Phimosis issue - the worst is if you only just see your Meatus (your pee-hole) and nothing of the head of your penis.
However, as you are describing it as Phimosis you must feel you have an issue with it, such as you are not fully able to expose the entirety of the head of your penis, or / and you experience Paraphimosis, for example? I wonder therefore whether you have tried any stretching techniques with your foreskin?
For comment on the rash you would need to retake the same photos in bright daylight or sunshine.
As well as, are you sexually active? Have you recently had unprotected sex? If not, how would you describe your personal hygiene?
