Whenever I have sex with my wife, or even just masturbate, my penis head gets inflamed and starts itching. The only treatment I've found to help is a prescription topical fluocinonide cream. With picture attached, any idea what is causing this and how to get it to stop/ treat it?

I am almost certain that this is a yeast infection. You both need to be on treatment and abstain from sex or both of you will keep giving it to each other again and again. You can go to a local Walmart and buy vagisil- its primary use is for women but like all advertisement it is universal. She will get this infection a lot more often than you will simply because of her anatomy and because of her anatomy she is 3x more likely to get infections, STDs utis than any man despite your circumcised status. She has to keep cleaning herself or shell keep giving you this type of thing
My wife had an abnormal Pap which required a leap procedure, so maybe we have just passed it back and forth. She’s pregnant now, so is vagisil probably a safe and effective treatment for us both? And for how long? I’ve used fluocinonide, and that tends to clear it up after a couple of days, but only until further sexual activity.
Also, would it be beneficial to get an adult circumcision for the long term since this is recurring?
Tang3 is a Urologist so he would best deal with the issues you have asked about. The general rule of thumb would be to get the vaginal infections sorted and there will be no problem for you.
Additionally, unless you have a severe issue with a foreskin it is always best to retain it as the benefits it provides far outweigh being circumcised …
