Well I already know I need to wear condoms and I know it's still a chance of spreading it to parts not covered by the condom so my question is wearing underwear during sex a preventative barrier? My outbreak was on the inside of the lip of the vagina so having the underwear on would keep it covered.

Your whole genital tract has the potential to shed virus when it is active, so any mucosal skin in the area (not just lesions) is a possible source of virus.
