IOL options for 45 yo very high myope

I had a vitrectomy for floaters almost six months ago which left me dizzy and disoriented. All tests and trials seem to point to the lens being compromised in some way, so it may end up I have the lens removed. I have been studying and researching options, but I can’t quite get the total hang of best options to try and keep some near vision.

i just turned 45 and started needing readers a few years ago but ignored it. These days i use 1.25/1.50 depending what glasses are near me at the time. I am a -11 in both eyes in contacts and -12 in glasses which i never wear. Uncorrected, my focal point is about three inches from my face. Otherwise i am totally blind, everything is a blur uncorrected. When i wear my contacts i can see pretty clearly from about two feet to infinity.

I have been looking at IOL options but not sure what the best route would be. I would prefer to sacrifice some distance vision to keep some near vision but not sure how it would work with my other eye. Is there a option that would balance me out with other eye? I’m not really keen on the multifocal for the reasons everyone says. I already see mild starbursts and halos due to my myopia and probably wouldnt want to amplify that.

so far i have looked at the 7mm IOL (name escapes me) the tecnis eyhance and the softport. I dont want Alcon lenses because their refractive index is too high and i dont want cat eye reflections.

Any suggestions appreciated. Sorry for typos.

The simplest way to get close vision is to use monofocal lenses in a monovision arrangement where the non dominant eye is under corrected by about -1.25 to -1.5 D. It is best to simulate it first with contact lenses. For example if you need -11.0 D in your non dominant eye, try using a -10.75 D lens to see how you tolerate it, and to determine how well you can read with it.

My optometrist just gave me -7, -8 and -9 trial contact lenses for my operated eye to see if blurring me down will help the dizziness, so it’s actually a good trial run. If I can at least tolerate -9 I’m in a good position for future IOLs. I use pilocarpine to help with the dizziness and it creates its own type of monovision with the pinhole effect it causes. It’s not very comfortable but I think it is mostly due to dim vision in the eye from the constricted pupil and not the acuity difference.
