Irritation on underside of penis

I’ve been dealing with an issue for about 5 months now for redness/ irritation/ rash located on the under side of my penis and near the urethra opening. I am 25 years old and the issue seemed to just start one day and has stayed the same or became slightly worse than when it started. It has been feeling irritated and it bothers me throughout the day. Some days and times it really bothers me while sometimes I don’t really notice the uncomfortableness, but I think it always feels worse than it looks. Also there are some days where it looks a more red then other days, and when I take it shower it looks the most red. Typically in the morning it doesn’t bother me so much, but throughout the day it bother me more and more. I have seen three separate doctors. My general physician recommended an over the counter hydrocortisone cream which didn’t make any difference. I then saw a dermatologist and was put on several different topical steroids, an anti fungal, and a protopic all of which did not make a difference over the course of the months. I’ve also went to a urologist and he did not know what to do or what was causing it, but when I was in the office I went through a test and Ureaplasma parvum was detected in my urine, which to my understanding is a low level bacteria. (although he did not think this was causing the irritation) I was put on doxycycline and then Levaquin to clear this up. I also did blood and urine tests and all STDs came back as negative. At this point I’m unsure what to do and have stopped putting and creams or medications on the area for a few weeks. I haven’t had any other issues besides the area being irritated like discharge or anything and the irritation is only in that one spot. It does not hurt when I urinate. I’ve been actively avoiding touching the area, but at this point nothing seemed to have worked or made any difference. I have attached a few pictures

Drew, before I give you a more considered response here is a few questions and comments:
Are you sexually active? If so currently, are you using condoms?
What I am most interested in is what is in contact with your penile skin on a daily / regular basis - this, literally includes everything: So you mention taking a shower - what cleansers, that is, do you use soap, shower gel and other body cleansers when showering?
Do you wash you penis during the day - if so, are you using soap? Do you ensure all your penile skin is completely dried after washing the penis, or following a shower?
How long did you use the hydrocortisone cream given to you by your general physician? Did you read the full instruction leaflet and use as directed?
Seeing a Dermatologist was a good idea - usually if you go back to see them and discuss the fact the what you have been prescribed to that point in time is not working for you they will work with you to find a solution.
Wise to get tested for STD’s - with the Urologist I think this was?
I wonder if you tried something like Desitin - which is for treating things like nappy rash?
