It's not any single thing plus he may also be looking for things other than prostatitis that could be causing your symptons. Chronic prostatitis is hard to diagnois and treat and often seems to come and go on its own.

I asked, and nothing scientific here, but I had significant intermittent bouts of prostatitis in my late 20's and 30's and sometimes in my 40's. But as I got older (I'm 70 now) the symptoms either went away completely or got much milder. Prostatitis can seem like it never will go away but I'm sure it will eventually get much better for you.

I can't answer how long it will last but I can say that most men will eventually find some relief, if not complete relief. Just try and work with your doctor, and don't shy away from getting a second (or even 3rd) opinion if you feel that you're not making progress.

Yes, it can take time to go. Also, try to remain hydrated and look out for triggers. For example, bicycle riding always made my prostatitis flare, so I either stopped riding for awhile or changed to a more prostate friendly seat. For some people it might be caffeine or alcohol or for the lucky ones, too much sex smile Anyway, good luck moving forward and let us know how things go.

It was long time ago when I was very symptomatic, but I'd say some of the flares lasted for several months. Even today, I probably still have some inflammation but the symptons are so mild that they don't effect me very much in terms of pain or discomfort. Just try and keep a positive attitude that it will get better because in most cases it will.

It's an individual thing, so what caused it with me in the past may not be a problem with you. That said, here's a short list from memory. Some of them may apply to you and some not.

bicycle riding

caffeine

alcohol

being dehydrated

spicy foods

too much sex 

too little sex 

sexual arousal without orgasm

some OTC drugs like antihistamines

With very bad flare ups I would often go on a course of antibiotics even without a positive urine culture. If the culture was positive (bacterial prostatitis) then I was definitely treated with antibiotics. The thing is that you really don't want to use antibiotics more than you have to.

Other than that, I tried symtomatic relief. Things like making sure I was well hydraed,  stop riding a bicycle, changing the bicycle seat, or avoiding a very long car trip, I also tried to avoid very spicy foods and cut down on alchol and caffeine. I found sitz baths (sit in a shallow tub of hot water) very helpful as well. 

Regarding urodynamics, you want to know what your PVR is, at what fill point do you feel the urge to urinate, what is your bladder pressure and flow rate, does your result suggest an obstruction, and if it does, do you have adequate bladder pressure. Also, ask if it shows any nerve damage. And always a good idea to get a hard copy of the report for your own records.

We all have different triggers and maybe sometimes there is no trigger. That in part is what makes chronic prostatitis so frustrating and difficult to treat. I know it's easier said than done, but sometimes the less we focus on things the better they get over time. Not to say you shouldn't watch your liquids and triggers but try not to let it overtake your thoughts and life because chances are it will eventually get better even if you do nothing.

Hi david,

You indicated "pain in bum", my guess is you mean pain in the butt (anus). Are you sure it is caused by prostatitis ? When my prostate is irritated, I usu feel it at the front lower abdomen. Maybe you have inflammation of the lower colon or hemmoroids ? Hank
