Its been like since about August so about a month and a half or 2 months no open sores no leaking or bleeding but it kinda stings or irritated daily by something it doesn’t hurt if I’m touching it or poking at it it only shows when my penis head is dry and is irritated after intercourse but as far as changes it has not been any just been the same ever since I noticed it back in August

its genital warts caused by hpv im sure. go look for a doctor for an early treatment

it is not genital herps. Most likely it is smegma, you should clean it daily with water and keep it dry and apply cold pressed coconut oil.
Does your meatus protruded like that since always?
