Need info on BPH Options

Large prostate (186cc). North of 70 a bit:) One case of balder stones. Tired of peeing every 1.5hrs Recent Dr. Bush MRI No PCa. Dr. Bush indicated he could help out, but all out of pocket:( Tried all the meds:( Stream is O.K. PAE sounds interesting, as does TULSA. Do not like the sound of TURP:) Want to limit "side effects" on any procedure (would like "Mr. Happy" to keep working). Looking for comments from anyone that has recently had any procedure done for BPH with LARGE prostate and thoughts. Also cost (did insurance pick up the procedure). Also DR / Locations recommendations to research. Other than some kidney stones from time to time, and the one case of bladder stones, no other issues. Just getting reeeealy tired of getting up every 1.5hrs to pee at night and the trips to pee during the day:) Live in L.A. That's Lower Alabama:) Thanks....

I have heard that PAE works with a large prostate, and it has few side effects. It is probably worth trying, because other procedures can be done after if results are not satisfactory. In the past PAE has not worked well with enlarged median lobes, so you should determine if you have an enlarged median lobe from your MRI or from a cystoscopy. In recent years some interventional radiologists say they can treat enlarged median lobes by getting further in to the arteries of the median lobe, but you should ask them about their results.

Dr Moule at Northwestern says PAE can be done on large prostates, also those with large median lobes. I have not gone to him so cannot vouch for him as such, but he sounds like he knows what he’s talking about. I’m pretty certain there are university centers in the south (Texas? Florida?) that do PAE. Also Aquablation claims to work on large prostates, and with medicare covering it more docs are going to offer it now.

Aquablation is kind of turp using water. Not sure OP is looking for that

I had a PAE 4 years ago, but prostate was “only” 98 cc. Got moderate improvement. If you opt for it, be sure to go with a very experienced IR. I went to VA (I’m in NY) to have it performed by Dr Sandeep Bagla. Incredibly good experience, all things considered.
