As Lostknight suggests, you need to see a Medical Professional about this. It looks like you have a skin disorder. If you can choose a Specialist go see a Dermatologist who specialises in Genital Skin Disorders, or a Urologist.
If circumcision is suggested (you don’t appear to be circumcised) my suggestion is that you try Steroidal treatment for 6 to 8 months first.
