Sounds like you are pulling on the muscles and tendons in arm and neck. The child needs to unerstand you are uncomfortable carrying Her around and it will make you ill.
I understand with Autism it is a condition where people need to be firm and kind. The request that She now walks has to be addressed. Is She able to still use here pushchair, some can be quite large.
All your daughter can do is cry when you refuse to lift her, You need to let this happen. The tantrum may last for a time and hopefully she will get the point Autism paitents seem not to get upset or take tizz the same way as a normal person
Be firm, very firm and be kind, Most kids like a carry if they can get away with it.
Your well has run dry. Be firm she will eventually understand you are not playing the game anymore

How is the Nursery getting around this problem. ?
I have only Had dealing with Adults with Autisim and I know we had to be really firm with them. Generally I found they fail to consider others although when told they have done wrong or are not considering others the best way was to put your foot down. They seem to accept the word No especially if you walk away. You could then try to divert Her attention and reward her in some way
You need to get some NSID medications for your arm and neck so some Nurophen may help there, that will reduce the swelling. You could also try NSID creams as well that will help with the soreness
The Nusery will need to be able to control this need ask for advice there as well. You are going to have to deal with this as it cannot go on as She becomes older She will get heavy and you will be in trouble.
Firmness is the only way here. Sorry

Talk to your GP regards this I can understand the Nursery and the Sessions she attends,
Have the scan and see how you go.
Your GP may be able to arrange some assistance by referring you to a charity or NHS Department who deal with Autism.
We had an adult sufferer near where we were and they had formed a team of Social Workers that could help. Try and see if you could get some form of treatment plan arranged, your GP should be responsive.
Let me know how you get on, you can PM me if need be
Good Luck
