It is a real positive that you have not had sex, as this means there are no sexual related issues mixed in with what you have going on.
I say this as I think you have several issues related to your foreskin, head and shaft of your penis:
The red and black looking spots you see could even have different causes, the red ones suggest a general unhealthiness of your penile skin, the black could be due to a long-term presence of moisture on your penis, and, perhaps entire groin region. When you have these circumstances it is easy for a fungal infection to take hold, these can be present on many areas of the body, the most common one is the central chest area.
The itching is most usual down to long-term exposure to soap and all forms of body wash such as shower gel - this is easily dealt with by using water ONLY to wash your penis with, and as I have mentioned the regular presence of moisture, what you must do when you wash the penis, or you take a shower is you need to completely dry your entire groin region with a clean dry towel that only you use. You need to pay particular attention to the head and shaft of your penis to ensure no moisture exists, prior to you getting dressed.
Then you have the swelling, you describe this as “ im having pain on my penis head as well as swelling in a circular manner ” and I am wondering whether this is specific to the corona / rim of the head of the head of your penis? However, the white patch you mention is not evident in the photos you have posted …..
Really, you have quite a lot going on here - you may benefit from a professional opinion on what you have and how these can be addressed. Is it possible for you to see a Urologist? Do you live in the UK or another country where you can access medical professionals such as this …… ?

thnx for your reply, i might add that i have pain on almost all of the head no specific region and when i was young i had phemosis and i still believe i have it now also and i am planning to see urologist. the only worry for me is cancer tho.

It is best to get a Urologist to check all the issues you have. What may be suggested is that you get a circumcision done, particularly if you have had phimosis issues for some years. If circumcision is suggested discuss conservative approaches rather than radical full removal of the foreskin.
If you can update your post here once you have seen the urologist that would be good …
