Could be an ingrown hair. Looks like what I used to get in my pubic area.
It certainly does look painful. It looks like an ingrown hair to me. I’ve had a few myself. Try hot packing it for about 15 to 20 minutes and then with a sterilized needle try to work the hair out and definitely get the pus out. Peroxide the needle and the area as you goes. The peroxide will also help dry out the infection.
The other thing you could try on it is a liquid antiseptic - if you are in the UK get some TCP and dab a bit on 3 or 4 times a day. TCP does not smell very nice but it sure does the trick, I use it myself on penile issues like this. If you suspect this is becoming infected obviously seek professional medical help, but something like TCP should prevent this …
