Aidan, I have had symptoms very similar to what you seem to be describing. Mine started way in excess of 10 years ago, and thanks to a referral to a Dermatologist I learned how to control my penile skin issues.
I subsequently found that issues similar to my own are very common in men (even more so in women), although the actual cause can vary, as can the outcome in terms of making the condition better.
I have a permanent incurable condition, other men (and women) can cure the issues they have simply by removing what it is that is irritating the skin of the genitalia.
The common thread is exposure to chemicals, and this can even be something as simple as soap and shower gel. This all becomes problematic because the skin develops a complete intolerance to anything that is placed on it - from normal skin creams and lotions, to even condoms then eventually your own semen and pre-ejaculate. Sometimes, even something as simple as exposure to clothing or towels washed in particular soap powders can further irritate skin that has become intolerant.
For anyone else reading this, the essential is that the only thing that should touch the skin of a penis that has become intolerant in this way, and exposure to anything else seems only to cause further irritation - is water. The is a very basic hygiene (and I mean hygiene in the broadest possible terms) essential - the second part of this essential is that once the penis has had contact with water (washing, showering after swimming for example) it must be completely dried - so for irritated skin that has scar tissue you must be very careful to pat it dry with a clean dry towel. If you even suspect there could be an intolerance to washing powder switch to Liquid Soap Flakes that you then use in a washing machine in the normal manner.
.
So back to your specifics Aidan, can you upload an image showing some of the most irritated skin tissue?

For others, whether male or female, if you have skin around the genitalia and / or anus (can be similar elsewhere on the body too) that simply will not heal the health Specialist you need to see is a Dermatologist specialising ion Genital Skin Conditions. For women, there are increasing numbers of specialists like this in Gynaecology departments - because as I say, the predominant sufferers or various skin conditions specific to the genitalia are women.
