Does anyone know what this could be ? I am concerned, and I am worried its been there for awhile and my girlfriend keeps asking me what is it but I keep saying I don't know && I'm embarseed , so does anyone know if this is a bad disease? Or is it just normal? I'm scared

Most of what you have on the rim of the head of your penis is Pearly Penile Papules (PPP) - very normal.
Not sure what you have showing at the very top of the photo - dried or dead skin perhaps … ?
A photo in taken in clear daylight helps …
