Hi I m 22 years old ..I do not know when is starts but from almost 10-12 years I have my foreskin attached with glance of my penis.... until now there is not a big problem.. but sometimes after urine discharge i can feel pain in one of vain of penis

And during urine discharge it look like a balloon(upper head of penis)

I have done sex not many but sometimes.i didn't not pain.

Can someone please help me what to do

Can you not retract your foreskin at all?    Being intact is cool, but phimosis is an issue.  It's certainly possible to live with a non-retractible foreskin, and many men do, but personally, I'd get it treated, both for hygiene reasons, and sexual enjoyment.  It's usually possible to treat phimosis with stretching or steroid creams, though occasionally preputioplasty or even circumcision may be required.  These links may help:

Also even though you still have your foreskin if you research foreskin restoration you will be able to find more info and support on the techniques that will hopefully allow you to treat your tight foreskin without having to resort to surgery.

Thanks for your reply....what they do in. A surgery... after that will the problem stop

That's difficult to say, since it's not clear if the pain is related to the phimosis.  Even if it is, surgery would usually be a last resort to fix it  You need to get it looked at by a doctor though.
