This looks like a patch of lichenified skin on the scrotum- your description of it being slightly itchy, leathery and smooth fits the bill. It’s also less hairy than the surrounding skin.
Lots of causes for this, including eczema, psoriasis, contact dermatitis, fungal infection etc. Lichen simplex is not a primary process. Rather, the underlying cause/s makes the person sense the need to scratch in a specific area of skin and causes mechanical trauma to the point of lichenification. So have you been been repeatedly scratching this particular area?
The best way of managing it is to find the underlying cause or trigger and then to treat that. This will stop you from scratching the area and the skin will then return to normal. If you can’t readily identify the cause, I suggest you see a dermatologist.
