Red spots on penis head

Backstory:
I had phimosis (tight foreskin) all my life, 1 year ago I began stretching exercises and soon managed to gain full retraction. However, I uncovered multiple red spots on the penis head.

I am unable to determine exactly how long they have been there, but I have known about then for 12 months and they have not changed in any way since then.

Occasionally the larger red spot (see attached photos) becomes painful, and after a short while (30s - 1 min) the pain subsides.

Originally I assumed it was balanitis considering the tight foreskin and lack of good hygiene. I have been maintaining good hygiene since I discovered the issue. I wash with warm water only (no soaps) and dry gently in the shower, and I dry thoroughly after urination. The hygiene improvement changed nothing.

Other treatment attempts:
I tried clotrimazole 1% cream and applied twice per day for almost 4 weeks, no change.

I tried fluconazole 150 mg oral pill 3 days ago and no change.

I am extremely concerned that these spots haven’t gone away, and the occasional pain can be frustrating. I fear that if it is a fungal balanitis then it is resistant.

I am not sure if it’s bacterial. And the last time I had sex was 3 years ago. Any guidance would be appreciated.

Thank you.
