Sharp pain between penis and large intestine post orgasm. Possible causes, advice on next steps?

When: Pain only occurs sometimes- specifically when I orgasm for the second time after not doing so for minimum of about 2 weeks (i.e. after 2 weeks of no org. the first org. is pain-free but the second one is not). If I orgasm again (3rd), the pain is there but weaker. Sometime by the 5th or 6th orgasm in a row, the pain is gone and as long as I maintain a frequency I can orgasm pain-free. First occurrence was when I first became auto-sexually active at 15 (now 22). Recently I found out that orgasm is not a pre-requisite. It may also occur when I take a dump right after maintaining an erection for about 40 minutes. This finally propelled me to start searching for answers. Where: Pain occurs behind the root of my penis, few centi-meters inside, feels like it is pushing into my large intestine, same area can be reached by going vertically up by about 2cm from a mid-point between my anus and testicles. The pain radiates outward from an area approximately the size of a 2-3 cm diameter ball. While in the post-orgasm situation the pain feels symmetrically distributed, in the case of it being brought on by pooping (after erection but no orgasm) the pain seem to be located mainly on the left side of the same area. The pain is almost always "complemented" with a perceived need to poop. Usually little to no poop can be produced at the time. With that second orgasm after >~2 weeks, I would rate the pain at 9/10. The pain lasts for about 30 minutes to an hour, with the first 1/3 of that time being at full intensity and the other 2/3 decreasing back to 0. I've been reluctant to approach doctors and did not consider it to be that much of a problem while younger but as I have more gaps between orgasms now, it has come to my attention again. I would be thankful for advice how I should handle this issue and I'm interested in learning the potential causes of this.
