Under my foreskin there has been some discolouration over time and it is a bit rough around the top of the foreskin.
Any idea what this could be?
Do you know how long ago this first started to look like this?
Just to check: the texture of the discoloured skin feels much rougher than the skin elsewhere on your penis?
Have you ever seen a medical professional about this? I am guessing you are in the UK? What age are you?

You need to get some treatment put in place for this as soon as possible as you may have quite a severe skin condition that can ONLY develop further without treatment and eventually the only option would be for you to get circumcised.
Contact your GP and if they are only doing phone consultations at the moment provide them with the photo you have posted here.
It is likely you will need to see a Dermatologist to get confirmation of what the condition actually is - if you have a main General Hospital within travelling distance you could go to a GUM Clinic, or you could at least phone them. Large GU or Sexual Health Services are more likely to have a Dermatologist that is part of the team, so this is an alternative to you contacting your GP

Somewhat mysteriously, my earlier post has disappeared. Apologies if you have already read it.
If not, I think you have features suggestive of lichen sclerosis- the discoloured (scarred) foreskin and the frenum that looks shorter than normal.
You should follow wee_dugie’s advice and attend a local sexual health clinic to have the diagnosis confirmed. Treatment at this stage would be a strong steroid cream like Dermovate, under medical supervision.
