this started a year ago. I noticed small bumps on penis head only when erect. now the pimples are more noticeable and spread around. no symptoms other than that no pain nor itching.

What you have is the skin of the upper shaft and head area of your penis has developed an intolerance to the things it is in regular contact with. This can be things like soap, shower gel and other bodily cleansers. You need to think is there anything else that you regularly apply such as skin cream, baby oil or moisturiser of any kind?
The pimples you can see are naturally present on the head and surrounding area of the penis, only, you do not normally see them until the skin is irritated and what you need to do is remove the source of irritation and they will no longer be visable. If you are circumcised this will also be complicating the situation as the natural way of keeping the skin of the head of the penis moist has been removed.
You need to wash your penis with water only, when you shower take some time to rinse away all body cleansers from the head area of your penis as these have become harmful to the natural balance of your penile skin. Always make sure you are completely dry after showering or washing your penis.
It will take quite some months before you notice any real change in how your skin will look, what you need to persist with is either excluding anything from getting onto your penile skin other than water and completely rinse away body cleansers when you take a shower.
