Solutions for complication after direct anterior hip replacement

I had the surgery in april and still can not lift my foot off the floor more than 2inches.  Continuous PT of various types since and to now.  surgeon kept saying just keep waiting for results, it takes time.  I am not class 2 narcotic with much side effects...and want the solution so I can get surgery or whatever to be back to normal.  Has anyone had this issue and what was your cause and treatment.....How are you now....How long did it take? Any input would be helpful.   thank you carole 89019

I wonder if it has anything to do with age? and reccuperation? I would again go back to the surgeon and ask for a more thorough diagnosis, or go through your GP asking the same question.

Carole89019, I would make appointment with gp and have them send you to neurologist you might have nerve damage , I had surgery in Oct 2013 when i woke up my foot felt weird I was referred to neurologist 6 months after my surgery and I have sciatic nerve damage from my revision thr in Oct 2013. Im not saying you have nerve damage but its possiable you might have foot drop. I wish you well and please let us know how you are doing

Hi Carole!

I also had my surgery in April and I had such a difficult time lifting my leg like you said. PT had me doing knee lifts (like marching) 3 sets of 10 twice a day. It seems like my leg has almost settled back into place. I'm still limping a ltttle but everybody says it takes time.

I would say to talk to your doc again and insist that he come up with a game plan for you.

Carole, you need a second opinion if all your doctor will do is say to wait. It is not normal to not be able to walk relatively normally by the two month mark. Something is wrong and your surgeon is not on top of it and offering solutions that will correct the situation.

So glad you found us on here and so glad you posted. This site literally saved me from giving up on life altogether due to my extreme bad situation and I had a surgeon who even wrote in his notes that "When distracted, patient seems to not be in pain." As if he thought I was making it up!

Lots of things can go wrong with surgery, any surgery is a risk. But please do not wait longer, something is wrong!

I am a strong self advocate and my motto is that I am a member of the medical team, in fact I am THE MOST IMPORTANT member of the medical team, so I have a say. I have rights. And I want you and everyone to adopt this truth, this attitude, because not all of us will luckily fall into the hands of the best medical team and sometimes one needs to stand up for what they want.

Your problem should be examined in every way possible so that you can find out what, why, and what to do next.

Dear Carole ... i am not sure if I understand - are you not able to lift your foot off the floor when you lay down, sit, stand or walk??  either way, it would concern me too, to be honest - where do you feel the weakness or pain when you try lifting your foot? 

Is there anybody who can come with you and speak for you? It can be so intimidating to face them and being told to wait ... making you feel as if you are a loser or something - (my experience) - 

Have you talked to your GP about this ... definitely go for a second opinion -

Hello Carole, so sorry to hear of your troubles, we all go thru some after op. niggles, but this sounds way out of the norm. I agree with Dawndee, your surgeon is at fault telling you to wait!! no no no, definitely no!! pain or inability to perform normal functions, IE. lifting your foot after 2 months, is a sign somethings wrong, go and get it checked out asap, don't be fobbed off with platitudes, you have rights, they have to listen.

With the direct anterior I was told I could go home and also I would move freely with moderate pain end of day after surgery.  I went to care center over 4 wks and PT at home until last week.  Now registered with Sister Kenney Rehab. who is also not sure what to do.

Had a scan for possible fracture or crack in bone and came out clear.

The leg will walk but I can not march even a little. BIG PAIN. Can not slip on shoe while sitting unless both hands pull it up.Foot will lift up about two inclhes with standing and forcing it while holding walker.

It does feel like more than one issue.  Hot stingy pain, sharp deep jabbing pain, numbness, cold feet, less swelling now .  My leg does not feel the same as the other when I touch it anywhere inside or top of thigh knee shin.  Comments have been suggest by PTs psoas, nerve damage, capsule or other types of tears.  I am interested in anyone who had psoas or any corrective type of surgery and is all better.  I am seeing a second opinion surgeon this week, but still hope to hear any input of successful fixes.  

thank you so much.  I have felt so alone.  On Tues I see a sureon for second opinion with actually only opinion. other than pt s comments of what they think.  i CAN NOT HELP BEING NERVOUS. thank you for the hug...I can not lift knee when sitting and can only lift two inches when I strain to do so while standing and supporting weight on walker.  Hope someone will know of anyone who has had this situation and can reply a successful treatment or surgery.  I am grateful for all

finally got appt for second opinion different surgeon. Been in pain too long  and i was told my age was not any issue especially for direct anterior as it supposedly is fast recovery with no or mild drug.  W r o n g in my case and I am thankful for the data on this site....it does help to not be alone.

Does anyone knw anyone who has made it through this type of thing.

It took a good 3-4 weeks before I could actually march and that was while I was in therapy.It was very difficult at first to stand on my leg because it did cause a lot of discomfort. But with continuing my exercises, my leg is almost back into the right place (still a little limping) but I do see improvement.

Carole: I had alot of trouble with my Psoas muscle and finally my surgeon told me  to stop PT and not do so much daily. He said that muscle needed rest, not PT. He was right. I am alot better now, but not completely healed. Could this be your problem?

I should also say that lifting the leg into the car, drivers side, is the "sign" that the psoas is at fault. I had to grab my pant and lift it. After a month of noPT and rest, I am alot better. Good luck to you.

Thisis what I have repeatedly posted here about the psoas muscle. You need to rest it, don't do any exercizes that make you go "ouch," don't aggravate it, let it heal. It took me a good solid 3 weeks, but I caught it right away. I couldn't sit for very long and particularly getting up from sittng for a while and then walking hurt it, so I didn't sit. I laid on the sofa or laid in bed.

My physical therapist diagnosed it, knew immediatly, touched the muscle and asked, "Is this it?" I sad "yes" he said that is your psoas muscle, so because I previously suffred with tendonitis if the IT Band(outside of your leg from your hip to your knee) I already knew the protocal was to rest  the tendon and not aggravate it. You can end up with a permenet condition where the little rips in the tendon/muscle develope scar tissue over them. I can still feel the scar tissue on my IT Band. Tendonitis is nothing to laugh about, you have to lay off the tendons because if you don't you potentially are going to have that pain for the rest of your life. I was AMAZED that the THR cleared up the tendonitis (and I was diagnosed that it had turned into tendonosis) of my IT Band. That was HUGE and actually more important to me than my bad hip.

My husband had excuciaitng persistant pain after an anterior THR and his x rays all looked great.  It was painful to sit and could not put on socks,  This was 18 months after surgery mind you.  We had just ordered crutches for bad days when I learned about Piriformis syndrome.  Doctors have to move that muscle to install the appliance and the sciatic nerve runs through it. There are lots of videos on you tube that explain how it looks and functions and exercises to improve pain.  It has made the pain bearable and he can function now.  There are two very easy nerve flossing exercise for piriformis syndrome on You Tube that he does on a chair and now he no longer uses a cane--a life saver,
