John, it seems you have a mixture of different things going on here. You say - " I have this painful irritation that only comes after sex or masturbation " and you also state - " it is painful at times and it stings after I pee and i feel like there is still pee to come out "
What you may have is a condition such as Urethritis or Bacterial Prostatitis for example. However, you also say you have a rash on your foreskin / penile shaft - pictures 1, 2 and 4 seem to show a redness patch just below the head of your penis. Is this much more distinct when you are looking at it?
Then you say about " a red line " do you mean the one that seems to follow what could be a blood vessel that is more distinct compared to, or simply different to other blood vessels?
Also you say - " I have been having problems since childhood with my foreskin " - can you detail what these are?
The issues with the itchiness you were experienced may have been a simple reaction to soap, shower gel that kind of thing you were using at the time - perhaps you shower very regularly?
You are not suggesting all, or several of these things are linked together as one condition?
I would suggest you need to concentrate on one issue at a time and the most distinct ones to get sorted are the soonest is the irritation after sex / masturbation AND the stinging sensation after urination. How is your daily hydration? It would not do any harm to drink 2 litres of water a day for a week or so and see if this helps. However, I would go to a Specialist Sexual Health Service, such as a GUM Clinic or a Walk-in city centre Sexual Health Service about the irritation and stinging sensation issues - do NOT mention everything you have going on until they are starting to treat you for the irritation and stinging - once that is in place say you have other stuff going on, particularly the rash you mention …
Simply, the Doctors you have seen may have simply been on ‘overload’ if you have stated everything you have going on with your penis and they can often only treat one thing at a a time …
