Photos are not in focus but both clearly show a single (?) pustule and some redness of the meatus (pee hole). Herpes is certainly a possibility (to cause the pustule), but chlamydia and gonorrhoea could also be present to explain the painful urination. Time to get checked asap.

hopefully its not herpes, but there are a total of 3 pustules. The ER said they think its herpes but won’t know for sure until the test comes back. After 24 hours the meds have had little to no effect. I will comment again when my test results come back.

I’d wait a little longer for the medication to work. I’m assuming ER gave you oral aciclovir for the presumed herpes infection. Unlikely to be a streptococcal (sore throat) infection.
