There's a card you can print off on the Department of Health site (I'd post the link but it wouldn't show up until Monday),, Google it

Yes it's a standard thing, oh I'll private message you the link (you need to be able to open / view Adobe PDF files)

Ask at your surgery.they can order a leaflet on splenectomy and it has a card in it.
they just order from the standard nhs orderline.
