Tecnis Synergy -- blurry at every distance and running out of options

I had the synergy implanted in my left eye end of August 2022. That eye was also treated with a laser to correct astigmatism. Early on it seemed that there was a “dark area” in my vision which dissipated. From the beginning, my vision in that eyes has never been clear. Through the pinhole device, my vision is better. But when reading the eye chart, the blurriness reigns. The retina specialist saw no problems. The eye isn’t “asking for” any lens correction. I do have to wear readers when working on the computer or reading. We have worked through any potential dry eye issues. My surgeon has no answers. Both he and my optometrist see a small amount of PCO but don’t believe it is enough to be causing this issue and it would have had to have been present the same day as my surgery. My second opinion doc said he would leave it alone, try a YAG laser because a small amount PCO adversely impacts a multifocal iol. He would not recommend an explant.
I’m late 50’s and not interested in having blurry left eye vision the rest of my life. If I was, I would not have had cataract surgery!
After delaying surgery on my right eye, I finally took the plunge hoping that my brain was struggling with the incongruity of the eyesight differential. I had the Symphony placed in the right eye and I have had zero issues with blurry vision. I have some halo effect but less than I currently have with the Synergy. With the Symphony, I am able to see from about 4’ to infinity fairly well and typically come in with a weak 20/20 and a good 20/25 unccorrected. Although according to the eye exams I can see in the 20/30 neighborhood with my Synergy, I feel like I am look though a cotton ball haze.
I’m currently wearing a contact over my Synergy to test the possibility of doing an explant of the Synergy and replacing with a monofocal aimed for close vision.
The Synergy has been a bust, I would not recommend it to anyone!
Anyone else out there struggle with this blurriness with the Synergy or having any thoughts on my predicament?

I think you have tried most of the standard things to resolve your issue, and I can’t suggest much more. The pinhole device is a quick way of determining if your refraction is correct. The fact that you can see better with it, suggests an improvement in refraction could be made.

Have you gotten an eyeglass prescription for your Synergy eye? It is usually the best way to find out if the power of the IOL is correct for you eye.

If you are thinking of having an explant make sure you hold off on having any YAG treatment. A lens exchange becomes much more difficult after a hole is cut in the bag with the YAG.

Thanks for your thoughts. I agree about the YAG, I am reticent to do that while considering a lens change. At this point, no correction seems to be called for. It is a bit weird. When looking at the eye chart, I feel as if I could see the letters more clearly if I was not looking through a “haze”. At this point I feel my options are: 1) do the YAG and hope it has a good positive impact knowing an exchange after that might no longer be an option. 2) Given my LE doesn’t seem to like the Synergy (blurriness, halos, starbursts) I could explant the Synergy and replace with a monovision lens. I’d likely have this aimed close in since it would be nice to limit “reader” use and be able to see computer. Given that I wore a contact for a few days to simulate near vision over my synergy and it didn’t bother me (other than that combo doesn’t lead to clear vision), this near monovision combined with my Simfoni could be a good combination.

A lens exchange is still possible after YAG, but is more difficult, and potentially limits the type of lens that can be used. The requirement for the haptics apparently are different due to the hole in the capsule caused by the YAG. You may be limited to a monofocal lens.
