Hi I'm male 34 years old. I have this problem basically for 6 weeks, i have tested for STDs and HIV and all negative. its very irritating and sensitive, every time I walk or I wear tight underwear it’s very uncomfortable, its driving me mad. Jamal, the hole is known as the Meatus. When were you tested for STD’s ?
Are you circumcised?

What you describe is usually caused by something that is irritating the skin - the clue could be from the tight underwear, so for starters try wearing looser ones for now.
In terms of the irritation it is most often linked to something you have recently stared using and this is in regular contact with the skin. So, this could be anything from a soap, a shower gel, or other form of body, say, that you use in the shower - or even, and this is where your clothes could be linked, to the washing powder or water treatments that are added to the washing machine when you clothes are washed.
