
To my untrained eye, it looks like your foreskin is fused to the glans. I have seen before (i’ve had foreskin problems myself through my life!) that if the foreskin is forceably retracted when boys are too young and before it’s naturally parted from the glans that it tears and then heals onto each other. That’s why, despite what you read online waaay too often by people obsessed with “cleaning”, you should never retract a babies foreskin until it’s ready to be retracted when their older, it’ll part when it’s good and ready too.

However, I think - if the photo is correct, your case is that the majority of your foreskin is fused to the head of the penis. This will not resolve itself, meaning you can take two approaches: i .) do nothing and hope no severe issues occur during adulthood. You should be aware that if you become sexually active one of the things you may develop is a Paraphimosis related issue where the lower part of the foreskin below the head of the penis swells up and can be painful in the extreme, so; ii .) Seek professional medical advice from your Family / Regular Doctor or GP who is then likely to refer you to a Urologist, or you could go directly to a GUM Clinic at your Local General Hospital if you are in the UK, and no, the do not just deal with sexual health problems, they deal with all Genital Health related issues.
Be warned, the advice you will likely get is that you need to be circumcised, what you should state very clearly is that you would simply like to have the adhered skin removed and therefore retaining the majority of your foreskin with all the natural benefits having a foreskin provides for your penis.
