Upper thigh pain after THR due to Avascular Necrosis

Hello, 

I am 57 yo female. Had Avascular Necrosis in my right hip. THR on 10/24/16. PT for 5 months. I develop severe upper thigh pain when bending over not squatting as I don't have enough leg muscle strength to stand up. Experience pain when standing up and walking after sitting or laying. Why more pain in upper thigh after bending.? I know it has to do with the muscle, as PT recommends a massage ball and rolling leg over 36" roller. I hear of great results from THR but not me. Sometimes I can stand for 8 hours with walking around but other times the pain is so bad I cry. Looking for suggestions, thoughts. I live in western MA so not sure of laws re: needles. All X-rays and MRI show nothing wrong. Thank you!! 

Hello, sorry to hear about the pain that you are experiencing.  We have some things in common. I also had Right hip replaced for AVN 2 years ago.  I am 55.  In my case, it never felt right.  I did some research and found that the outcomes for THR for AVN are not as good as the outcomes for those that have arthritis as the underlying cause.  In my case it felt like a knife all the time in the back of my hip.  I pursued with the MD and went back for exploratory surgery 7 weeks ago and that pain is gone.  I am having other pain in my knee, but since not doing anything for a couple of years this will work out over time.  In my case the MRI showed a few things, but not everything and it wasn't until they opened me up that they saw things that were not on the MRI.   Xrays and MRIs are  not good to show soft tissue problems.  In my case the IT band had separated and had to be fixed, but this did not cause me any pain in the thigh area.  Have you been back to the surgeon?  Do you have a top notch PT?  this can make a huge difference.  I was seeing one PT with alot of degrees and education and I did not have confidence in her.  She did accupuncture and then couldn't get the needle out. She told me that the avn was arthritis and gave me alot of painful stuff to do, so I moved on.  My current PT was recommended and she is excellent.   Best of Luck.  Leah

Unfortunately I am not able to answer your question why your upper thigh hurts so much when you bend over ..

I would think that perhaps the stem has moved a little bit but that would have shown on the x-ray and MRI -  

How long do you have this ?  

Leah might be right - AVN is different from Osteo Arthiritis (duh, right?) -

is there any plan of treatment ?

Please let us know how you are getting on otherwise

I am so sorry that you are in pain. I can relate.  I am 55. 

I broke my hip when you’re ago (femur head went thru the hipsocket),January 11. 2 days later the socket was repaired From there I developed necrosis in the femur head and believe it or not I injured my shoulder using the walker. The femur head had AVN and September 1st I had a hip replacement Halloween I had a shoulder as a result of the walker related shoulder injury. The THR replacement was going OK at first but after six weeks I started to have a some pain. I’ve been diligent with my rehab but now the pain has gotten continuously worse and I am so frustrated. 

Yesterday I had an MRI and I am looking forward to the results which I will probably get tomorrow.  We’ll see what’s going to happen. I know something is wrong inside there. I did read below where the one person said that they had exploratory surgery and honestly I am so ready for that now. It’s been an entire year of struggles and pain and I just want to be able to walk again.
