not sure what this is, itches sometimes. hx of a lot of skin tags.

It must be an anal fissure (hemorrhoid), the itching is typical of the hemorrhoid

It is a hemorrhoid not a fissure. The skin tag is leftover from when the hemorrhoid thrombosed now that it has gone down you are left with the skin.
