Sorry this is long, but let me get this all off my chest and maybe there’s some tidbit’s in here that can help us both find an answer. I thin this is a very specific thing that doctors just don’t know enough about.
First.. Thanks for responding. Lets compare notes as it seems were the only to people online with these bizarre issues. I’ve googled like crazy and can’t seem to find anything relating to this. I’ve been apprehensive to start going to doctors because I’m not sure I know how to explain it well enough. Let me tell you what I’m experiencing and when it started etc.
In January I went to the urologist because I had this weird ammonia smell emitting from my armpit and some abdominal pain (internet told me it might be my kidneys.) Those things cleared up by themselves eventually but while I was there he tested my prostate and said it was a little “boggy” - he ordered a PSA (prostate cancer test) and told me it was probably Prostatitis and that it would clear up by itself. Unfortunately I had to wait 2 months to receive my PSA results and was under an enormous amount of stress thinking I had prostate cancer. (did I mention I’m a bit of an anxious bloke?)
The one thing worth mentioning is that during this entire time (2 months) I didn’t have ANY prostate symptoms (frequent urination, pain, burning etc.) I felt great… except I was super stressed about my PSA result.
Anyway, mid February about a month and a half after my urologist visit i muddied up the waters big time. This is embarrassing to admit, but while I was getting oral sex from my wife I squeezed the base of my penis to make the head expand more and held it for a few minutes. It didn’t hurt while I was doing it but there was lots of pressure. When I ejaculated it felt like 4 electric shocks going off after each other in my shaft. The next day my penis was very retracted and injured looking, kind of like when you are young and you play with yourself too often. I had constant sensation in my penis and I felt like I was dribbling constantly/wetting myself. I was also feeling dribbles hitting my leg.
2 weeks later I finally had my follow up with urologist - he informed me that my PSA was only 0.9 and that my abdomen ultrasound came back fine. I told him what had happened since last seeing him and he said that I may have injured something in my penis, but that the symptoms I was reporting sounded like prostatitis still and I should take a course of antibiotics just in case.
It was two weeks into my antibiotics when I realized my dribbling wasn’t actually real!!! I felt a drop hit my leg and realized, there’s no way that could have hit my leg, so I inspected my leg and my penis and found they were both completely dry!!! Sound familiar? So from that point on I now understood that I wasn’t actually dribbling but I was getting the SENSATION of a dribble/wet penis. It feels exactly like you describe, like there’s a drop of pee at the end of my penis but its bone dry. I have sometimes wondered if I’m confusing cold for wet, but WET is definitely what it feels like.
If I lay down flat it all goes away. In fact its usually not there in the morning but slowly comes on as I’m up and about and moving around. Its worse if I sit with my knees up (to me that seems like there’s pressure on something)
I don’t have the tingling balls like you describe or the pain in my bum or legs (yet) but I do have a weird feeling that my poo isn’t the same, like its more difficult to push out, almost like I don’t have the same strength.
I am feeling a little lower back pain. I’m also getting what feels like cold water drops randomly on my body. My legs my neck, arms, feet, chest, face sometimes.. everywhere. This is the symptom that has stressed me out the most. When I google it, its called paresthesia and its usually from nerve damage (MS, Diabetes , and B12 deficiency causes nerve damage.. and maybe squeezing your penis too hard, lol) So of course now I’m constantly worried I have MS and I’m in perpetual misery.

None of this was happening until I squeezed my penis in February and felt those electric shocks. So I’ve assumed its related to nerves , that I maybe damaged a nerve in my penis and now my body is misreporting the sensation of wetness. Especially since I’m having these body sensations. Its super ironic that I went to a urologist ahead of all this and he suspected prostatitis, but as I said I had none of the symptoms associated with that.
Anyway, I’m not sure if you’ve looked into the prostate side of things or the nerve side of things but id be curious to know what tests you’ve had thus far and I’m contemplating my next move.. either general doctor, urologist or neurologist.
Have you tested for diabetes or ms, or b12 deficiency? Had an MRI or any x-rays etc. How’s your prostate? I read diabetes can cause penis issues due to nerve damage (I’m fit and healthy like you so diabetes seems way off my radar but i think its worth checking)
I do get a sense that some of this is stress related as I was under a LOT this past 6 months, but I can’t omit the boggy prostate or the bedroom injury.
What’s weird for me is, if I drink a bottle of wine all my symptoms go away, no dribbly penis or body zaps. Obviously I cant just stay in a drunken stupor the rest of my life, but I find it interesting that it goes away when drinking alcohol. People say it must be stress then, but honestly i think I’m just numbing things with the booze cus it all comes back the next day and I’ve worked really hard on my anxiety levels to no avail.
So , is there anything in here that sounds familiar. I definitely see some parallels (dribbles that aren’t there, tingles etc)
